Should podiatrists refer all post-amputation patients to a pedorthist or orthotist for conservative care in the form of offloading devices? That was the shared opinion of an expert panel in a recent article which appeared in Podiatry Today. In the article titled “Current Perspectives On Post-Amputation Management,” all of the panelists agreed that “a referral to an orthotics and prosthetics professional is a crucial post-amputation intervention.”

As a practicing podiatrist for 40 years, I have seen thousands of patients. About 33 years ago, I became frustrated with the fact that although many of my patients responded to conventional treatment, many did not, and I wanted to know why. To me, this was the missing link in improving my percentage of successful outcomes. My traditional podiatry training taught me that in the world of connective tissue disorders, absent frank injury, we were dealing with inflammation that likely was borne out of biomechanical dysfunction.

Knee osteoarthritis (KOA) is a degenerative progressive joint disease characterized by chronic joint pain and stiffness, leading to the limitation of daily living activities and physical function. KOA is estimated to affect 18% of adults over 45 years of age and is a leading cause of functional disability. Aetiology of KOA includes traumatic injury, genetics, obesity, and poor joint biomechanics, with poor biomechanics a likely cause of primary progressive KOA.
