The Case Management Society of America (CMSA) is partnering with PRIME Education to offer a free CME educational activity designed to enhance primary care team effectiveness in value-based care models.
“Optimizing the Primary Care Team for Success in Value-Based Care” provides physicians and healthcare providers with critical insights on integrating case management into their teams. Through evidence-based strategies, participants will learn how case management plays a pivotal role in improving patient outcomes, enhancing care coordination, and driving financial sustainability.
The CME is available on PRIME’s website and will remain accessible until January 2026.
