Change Management: The Future of Care Coordination and Integrated Care

The future of case management requires leadership skills with an awareness of how to communicate changes and the benefits to the client. This presentation will provide key strategies related to communication of organizational changes, managing resistance, and obtaining key stakeholder buy-in. In this presentation, the participant will be able to define the change management models related to various organizational care delivery systems to discover the potential for improvements in health equity, care, outcomes and satisfaction.
