High Medical Complexity patients are individuals who survive a life-threatening illness or injury with or without multiple, co-morbid conditions who are debilitated and dependent on intense medical care for a longer period of time. Critical illness recovery care follows acute intensive care and is provided using a multidisciplinary, system/organ preservation and conservation treatment approach. Referral timing is critical when navigating patients through the continuum of care and the transition to the next level of care. Getting the right patient in the right level of care at the right time is critical to throughput, to optimize patient outcomes and maximum recoverability.
