Patients with pulmonary arterial hypertension (PAH) often undergo multiple transitions throughout the course of their disease, beginning with the journey to achieve an accurate diagnosis and initiate appropriate therapy. The complexity of the disease and therapy requires ongoing interprofessional coordination and system-based approaches to effectively manage care transitions to minimize hospital readmissions and optimize patient outcomes.
In partnership with the National Transitions of Care Coalition (NTOCC) and Case Management Society of America (CMSA), a transitions of care white paper has been developed for system leaders and interprofessional clinical team members who treat and manage patients with PAH. The guidance paper, developed by a multidisciplinary advisory board, incorporates essential interventions and pathways designed to ensure effective and safe transitions of care for patients with PAH across healthcare settings.

CMSA and the board of directors are pleased to offer CMSA's Standards of Practice for Case Management, 2016 revision, which provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016’s revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager.
The 2016 Standards contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

program, Comprehensive Motivational Interviewing Training for Health
Professionals, is now available. This training uses a sense-making
approach (Berger BA and Villaume WA, Motivational Interviewing for
Health Care Professionals: A Sensible Approach, APhA Press, Washington,
D.C., 2013) to motivational interviewing. The program can be completed
within a three-month window and is approved for 8 hours of nurse,
pharmacist and physician continuing education credit. It is based on the
authors' 25 years of experience in teaching motivational interviewing

Every year the Office of the Actuary at the Centers for Medicare & Medicaid Services (CMS) releases an analysis of how health care dollars are to be spent in the decade ahead. According to CMS’ analysis for 2014 (the last available report of annual data) the National Health Expenditures (NHE) reached $3.0 trillion or $9,523 per person. That expenditure includes the cost of all health care goods and services, public health activities, government administration, the net cost of health insurance, and investment related to health care. Although the final 2015 NHE data is not yet available, CMS projects they will reach $3.207 trillion. With the U.S. Population hovering around 320 million, that means this may be the first year that healthcare spending will reach $10,000 per person.

On July 7, the Centers for Medicare & Medicaid Services (CMS) proposed changes to the Physician Fee Schedule to transform how Medicare pays for primary care through a new focus on care management and behavioral health designed to recognize the importance of the primary care work physicians perform. The rule also proposes policies to expand the Diabetes Prevention Program within Medicare starting Jan. 1, 2018. This is the first time a preventive service model from the CMS Innovation Center would be expanded into the Medicare program.

The Health Resources and Services Administration (HRSA) announced more than $149 million in new awards through 12 workforce programs to prepare the next generation of skilled, diverse primary care providers to serve communities in need across the country.
"These awards will help increase access to quality health care for all Americans by educating and training culturally competent providers who are prepared to practice in high-need areas," said HRSA Acting Administrator Jim Macrae. "By encouraging partnerships among academic institutions, clinicians, health care sites and public health entities, we can improve health outcomes in underserved communities."
