In continuation of CMSA's 2014 Webinar series, CMSA will host the following webinar:
Transitions of Care Guidelines for the Pulmonary Hypertension Patient Tues., Oct. 28 12 p.m. – 1 p.m. CDT Presented by Sharon Jones, RN, MSN, CCM, Nurse Navigator, Christiana Care Hospital Pulmonary Hypertension (PH) or high blood pressure in the lungs is a progressive, incurable disease that is a challenge to treat; and an oftentimes stressful journey for patients and all involved in their care. PH patients are very often misdiagnosed initially, since the signs and symptoms of the disease mimic others that are better known. When the diagnosis is made, PH patients require specialized treatment by trained medical personnel who are well-versed and familiar with prescribing their costly life-saving medications.
This presentation will provide guidelines and best-practice information for case managers in a variety of settings that clearly outline their roles and responsibilities so that safe transitions of care for the PH patient can be realized.

Case managers are burdened by heavy workloads, numerous tasks, and demanding time frames. In addition, they need to have a vast amount of knowledge about a myriad of health care topics (e.g., conditions and diseases; insurance and benefits; available financial and community resources; treatment protocols; clinical standards and guidelines; medications, including dosages, indications, and side effects; and many, many more). It is impossible for nurses to memorize everything they need to know. The constant pressure to provide the best possible care and stay up-to-date with current knowledge and practices is a daunting task for even the most qualified case managers.

Premiums, copays, and deductibles for other Medicare programs for 2015 also announced
Secretary of Health and Human Services (HHS) Sylvia Burwell announced on Oct. 9 that next year’s standard Medicare Part B monthly premium and deductible will remain the same as the last two years. Medicare Part B covers physicians’ services, outpatient hospital services, certain home health services, durable medical equipment, and other items. For the approximately 49 million Americans enrolled in Medicare Part B, premiums and deductibles will remain unchanged in 2015 at $104.90 and $147 respectively. This leaves more of seniors’ cost of living adjustment from Social Security in their pockets.
According to the HHS Office of the Assistant Secretary for Planning and Evaluation, as compared to Congressional Budget Office (CBO) projections for 2015 made in 2009, premiums will be more than $125 lower over the course of a year.
