CMSA's Standards of Practice for Case Management serve to drive best practice accountability for individual professional case managers as well as for organizations.

CMSA's new online course is designed to enhance understanding by expanding on these common principles to deliver a more unified education to professional case managers creating quality skill sets across the healthcare continuum. The coursework embraces the case management care continuum in any work environment, which contributes to consistency in foundational case management knowledge and a sustainable workforce.

The course is made up of 16 online modules, each representing the sections of the CMSA Standards of Practice, including one on each of the 15 Standards. Each module contains a detailed narrative with enhanced explanations of that Standard including bibliographies, references, and a professional video presentation of that narrative content with downloadable slide handouts, which incorporate case scenarios showing real-life situations in order to better understand that Standard. This course is pre-approved for 32 hours of continuing education credit for RN, SW, and CCM.

CMSA's Standards of Practice for Case Management, 2016 revision, provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016 revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager.

The 2016 Standards of Practice contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

In 2011, the U.S. Department of Health and Human Services (HHS) released new rules under the Affordable Care Act (ACA), aimed at helping doctors, hospitals and other providers better coordinate care by way of accountable care organizations, or ACOs.

ACOs provide a framework for rewarding providers financially — in both the public and private sectors — for ensuring that patients, especially the chronically ill, get the right care at the right time, while avoiding unnecessary duplication of services and preventing medical errors.

Since then, the number of ACOs has grown dramatically.

Under the Centers for Medicare & Medicaid Services (CMS), ACOs agree to manage all the healthcare needs for a minimum of 5,000 Medicare beneficiaries for at least three years. For ACO guidelines, CMS established 33 quality measures focused on care coordination and patient safety, the appropriate use of preventive health services, strengthening care management for at-risk populations and improving the experience of care for patients and caregivers. Savings from this initiative are expected to be millions of dollars.

On April 2, the Centers for Medicare & Medicaid Services (CMS) finalized polices for Medicare health and drug plans for 2019 that will save Medicare beneficiaries money on prescription drugs while offering additional plan choices.

"The Trump Administration is taking steps for seniors with Medicare to save money on prescription drugs," said CMS Administrator Seema Verma. "The steps we are taking will drive more competition among plans and pharmacies to meet the needs of seniors and lower costs."

Around 800,000 people experience strokes in the US every year, and nearly 25% are recurrent events. A major risk factor for ischemic stroke is atrial fibrillation (AF). For people with AF, stroke risk can be reduced by 60-70% with anticoagulant medication. However, the recent PROSPER study revealed that 84% of more than 94,000 patients with AF who were admitted to hospitals for acute ischemic stroke had not been receiving guideline-directed therapeutic anticoagulation.

PCORI's dynamic video activity is designed to support healthcare professionals, patients and caregivers, and other stakeholders in applying key findings from the PROSPER studies to improve outcomes for patients with AF and stroke risk. The expert panel features a lead researcher and a patient-investigator on the PROSPER team; clinicians who represent the interests of peers in primary care, cardiology, and pharmacy; and a nurse case manager, who addresses key issues in care coordination for ischemic stroke prevention and recovery. Five short companion videos feature focused discussions on specific applications of the unprecedented insights into anticoagulant medication use and patient-centered perspectives revealed through the PROSPER studies.
