Care Coordination and care transitions are key ingredients in the national agenda to improve the quality and affordability of healthcare in the US. According to HHS, patient-centered care improves the patient experience and long-term health outcomes. To deliver patient-centered care, we must adopt CMSA’s Standards of Practice, most specifically the ethics Standard. A sound care coordination or transition model involves the patient and family/caregiver recognizing and attending to their needs and preferences.

 

Objectives:
As a result of this program, the participant will:

• Accept the ethical obligations for care coordination and transitions of care
• Recognize unethical practices and know where and how to report
• Learn the definition of patient-centered care
• Become familiar with the models of care coordination and transitions
• Learn the appropriate measurements used to evaluate the success of care coordination and care transitions

Significant advancements in multiple sclerosis care, including the arrival of new therapies, have made treatment decision-making and navigating health insurance requirements increasingly complex for patients and their providers. To simplify the process, PRIME worked in partnership with the CMSA and the Academy of Managed Care Pharmacy (AMCP) Foundation to develop myMS.care, a new online tool featuring 2 complimentary web apps. The web apps – a CarePath for patients, and a CareNavigator for providers – are tethered to facilitate shared decision-making and help guide critical conversations between those receiving care and those delivering it. Aligning preferences and goals to ensure patients and providers are on the same page often leads to greater satisfaction and improves the patient experience throughout the course of the MS journey. By offering specific tips on efficiently navigating the insurance process, the web apps are instrumental in avoiding unnecessary delays in treatment.Additionally, patients using the web app are able to print a customized "care guide" to take with them to their medical appointments. The care guide, which includes a list of the patient’s preferences, key concerns, and questions, serves as an important resource and reference tool when patients are deciding on a care plan with their providers.

Adverse events have been tracked for years, but mostly as they relate to issues that occur during a hospitalization, or during drug or clinical trials.  Adverse events refer to harm from medical care rather than underlying disease and are defined as:

- "An injury that was caused by medical management (rather than the underlying disease) and that prolonged the hospitalization, produced a disability at the time of discharge, or both."
- "An unintended physical injury resulting from or contributed to by medical care (including the absence of indicated medical treatment), that requires additional monitoring, treatment, or hospitalization, or that results in death."
- “Any unfavorable and unintended sign (e.g., an abnormal laboratory finding), symptom, or disease temporarily associated with the use of a drug, without any judgment about causality or relationship to the drug.” (Used in drug and clinical trials)
- An injury that occurs due to “negligent care that falls below the standards expected of clinicians in the community.”

While numerous studies have been conducted to identify adverse events occurring during hospital stays and trials, there are only a few that address adverse events that occur after a patient is discharged from the hospital and transitioned to home or an outpatient facility. Because the process of transitioning a patient from a hospital setting to home can lead to patient safety and quality issues if not done correctly, more emphasis needs to be placed on trying to study and identify the causes for post-discharge adverse events.

Centers for Medicare & Medicaid Services (CMS) Administrator Seema Verma in November 14 announced that the agency has taken steps and is monitoring conditions in support of California residents displaced and recovering from the wildfires destroying portions of the state. On Nov. 13, 2018, Health and Human Services Secretary Alex Azar declared a public health emergency (PHE) in areas of California retroactive to Nov. 8, 2018. The PHE allows CMS to waive or modify certain Medicare, Medicaid and Children’s Health Insurance Program (CHIP) requirements if necessary to provide health services.

 

“The lives and homes of thousands of Californians are at risk from dangerous wildfires and CMS is taking the necessary steps to provide flexibilities that help meet the medical needs of all those who are affected,” said Administrator Verma. “We are committed to helping everyone who has been impacted as they recover and rebuild their lives by ensuring access to care for themselves and their families.”
