The Case Management Society of America Foundation (CMSAF) is offering a scholarship for the Spring 2019 Integrated Case Management Training Program, presented by the Case Management Society of America.

 

CMSAF is offering this scholarship to a case manager who is committed to improving the health outcomes of the most at-risk populations and is willing to link and integrate care coordination across all domains of health. At-risk populations include individuals who have multiple chronic conditions, the frail, elderly, children with special health care needs, those with co-morbid medical and behavioral conditions, and may include the presence of social or health system barriers. CMSA's Standards of Practice for Case Management, 2016 revision, provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016 revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager. The 2016 Standards of Practice contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

We are all familiar with setting goals. It is something we do daily helping our patients/clients set their healthcare goals and something we do on an annual basis as we set our work performance goals. But how often do we really think about the overall goal-setting process and how it relates to not only our work life, but to our personal life. I would venture to guess that most of us take a lot more time and effort defining our work-related goals than we do our personal goals. Work-related goals are usually done in a much more formal, written format, whereas personal goals are more often than not a one-line general statement that is usually not even written down. Let’s take a look at the formal goal-setting process and see how it can be used to help us achieve some of our important personal life goals, as well.



The following are definitions for a GOAL:  

-An intention or purpose
-The object of a person's ambition or effort; an aim or desired result
-The end toward which effort is directed – an AIM
-An idea of the future or desired result that a person envisions, plans and commits to achieve
-An observable and measurable end result having one or more objectives to be achieved within a more or less fixed timeframe

All professions have unique skills, knowledge, and ethical standards that guide the professional in what knowledge and responsibilities are required and how to execute what has been learned. Three features characterize a profession and professional values:

 

1. Specialized training

2. Recognition of the need for standards of practice

3. Commitment to provide a service that goes beyond the personal interests of the professional

 

These values are critical to the ethical identity of the professional. The ever-volatile healthcare environment requires healthcare professionals to be guided by values and principles. The level to which we identify as professionals includes academic level, years of service, participation in professional organizations, serving in management positions, type of work, and type of work environment.

Supporting ambulance triage options aims to allow beneficiaries to receive care at the right time and place 

 

Today, the U.S. Department of Health and Human Services (HHS), Center for Medicare and Medicaid Innovation (Innovation Center), which tests innovative payment and service delivery models to lower costs and improve the quality of care, announced a new payment model for emergency ambulance services that aims to allow Medicare Fee-For-Service (FFS) beneficiaries to receive the most appropriate level of care at the right time and place with the potential for lower out-of-pocket costs. 

 

“This model will create a new set of incentives for emergency transport and care, ensuring patients get convenient, appropriate treatment in whatever setting makes sense for them,” said HHS Secretary Alex Azar. “Today’s announcement shows that we can radically rethink the incentives around care delivery even in one of the trickiest parts of our system. A value-based healthcare system will help deliver each patient the right care, at the right price, in the right setting, from the right provider.”

Accessed from the same landing page and requiring no download, patient-focused CarePath and provider-focused CareNavigator help align preferences and goals, ensuring that patients and their providers are on the same page to help improve the patient experience and outcomes. These online tools are also designed to help simplify the process of navigating increasingly complex health insurance requirements for both patients and providers by offering specific tips to avoid unnecessary delays in treatment. In addition, patients can print a customized “CareGuide” to take with them to their medical appointments. The CareGuide, which includes a list of the patient’s preferences, key concerns, and questions, serves as an important resource and reference tool when patients are deciding on a care plan with their providers.
