The course briefly reviews spinal anatomy, pathology, and types of spinal fusions. The subject matter covers the clinical and medical management challenges of difficult-to-heal bones, including the spine. Risk factors that negatively impact healing are reviewed. Therapies, both conservative and surgical as well as current trends, outcomes, cost, and patient convenience are discussed. CMSA's Standards of Practice for Case Management, 2016 revision, provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016 revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager. The 2016 Standards of Practice contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

They need to have a wide-ranging knowledge base that includes all types of diseases, conditions and procedures in order to educate the variety of patients they help every day. In addition to educating their patients and caregivers on the specifics of a disease, condition or procedure, they also help patients develop a customized care plan, assist them in navigating the healthcare system, provide information about resources, investigatepossible financial assistance that may be available and do whatever else needs to be done. That requires an immense amount of knowledge. This type of information is also needed by staff educators for the initial onboarding of new case managers and for routine ongoing staff education. In addition, management staff also need information like this, as well as access to best practices, case studies, surveys and other tools to improve current programs or develop new initiatives. Now, in the technology and internet era, there is no lack of sites that can provide that type of information, but it takes time to research it, not to mention additional time to organize and save it for later use.

This is true for patients with congestive heart failure (CHF), which is among the more serious conditions for patients and their families. It is in these cases that the role of the case manager and the implementation of best-in-class practices are critical. As part of that role, there are many dynamics at play, including communications with the patient and family members to assure adherence to the treatment plan, communications with other professionals across multiple disciplines on the care team, ongoing patient follow-up, documenting the patient’s progress and striving to prevent unnecessary hospital readmissions. Understanding how best to meet the needs of CHF patients and help achieve optimum patient outcomes while containing costs is important for all case managers.

Final Rule will strengthen popular Medicare private health insurance plans, expand telehealth access for patients, and improve coordination for dual-eligible individuals On April 5, the Centers for Medicare & Medicaid Services (CMS) finalized policies that will increase plan choices and benefits, including allowing Medicare Advantage plans to include additional telehealth benefits. These policies continue the agency’s efforts to modernize the Medicare Advantage and Part D programs, unleash innovation and drive competition to improve quality among private Medicare health and drug plans. “Today’s policies represent a historic step in bringing innovative technology to Medicare beneficiaries,” said CMS Administrator Seema Verma. “With these new telehealth benefits, Medicare Advantage enrollees will be able to access the latest technology and have greater access to telehealth.
