DMV Has Streamlined and Revamped the EPN Application Process and Forms

In an effort to improve customer service, simplify, and enhance the Employer Pull Notice (EPN) application process the Department has revised, created and obsoleted various EPN forms.
NEW OR REVISED EPN FORMS
• The Employer Pull Notice Program Requirements (INF 1107 NEW 7/2018).
• The Employer Pull Notice Program Enrollment Application form and Instructions (INF 1104 REV. 8/2018).
• Commercial Employer Pull Notice Enrollment or Deletion of Drivers form and Instructions (INF 1100 REV. 8/2018).
• Commercial or Government Employer Pull Notice Enrollment of Out-of-State Licensed Drivers form and Instructions (INF 1102 REV.• 7/2018).
• Employer Pull Notice Account Approval Letter (INF 1304 REV. 7/2018).
• Employer Pull Notice Change of Account Information form and Instructions (INF 4 REV. 7/2018).
• Request for Closure of Employer Pull Notice Account form and Instructions (INF 1112 NEW 10/2018).

• INF 1105 Employer Pull Notice Agreement (Obsolete as of 9/2018).
• INF 1250A Employer Pull Notice Program Enrollment Packet (Obsolete as of 9/2018).
Once an account is processed and approved an EPN Account Approval Letter, containing the EPN Requester Code will be mailed to the customer. This letter will be used as verification of EPN enrollment and must be maintained by the customer.
A minimum of one driver must be enrolled, and can be added on the EPN Application (INF 1104 REV. 8/2018). The $5 enrollment fee for each driver must be submitted with the application.
Customers will no longer submit the EPN Agreement (INF 1105 REV. 6/2014); this form is now obsolete.
Effective March 1, 2019 outdated forms will no longer be accepted
