DMV Driving Records - "H6" or "10 Year Record" No Longer Available

Effective March 20, 2019, driver records formerly referred to as an "H6" or "10 year record" that included information intended for internal department purposes are no longer available. Driver records will be on the K4 printout and will only include all reportable information: convictions for three, seven, or ten years, departmental actions, and accidents on a person's driver record as required by California Vehicle Code §1508.
Driver records can be requested online for $2.00, by clicking here. Note: The online driver record is not an official document. If you want to request an official document, fill out form INF1125, pay the $5.00 fee, and mail at to DMV Headquarters. The INF1125 form is available online or at DMV Field Offices. We are verifying with the DMV if the K4 printout should be used to fulfill the motor vehicle record requirement for the Driver Qualification File and will update you when we have more information.
